On March 24, 2004, the Academic Senate approved a form as an addition to the new program approval process. This Financial Implication Form (or Word), verifies that the financial implications of any new program have been examined and that resources are available for the implementation of the program proposed. Please complete this form for all new programs and submit it as you would all other forms relevant to the approval process.

Request to Add Hours Form
The Request to Add Hours Form Proposals includes information concerning how the program will be financially supported to proceed through the curricular process, as an increased total hours required in existing undergraduate and graduate programs (degrees, sequences, minors, and certificates) will be added to the curriculum.
